A licence is the first question. Certification is the next one.

Knowing which state will license you does not tell you which specialty board will certify you afterwards — and for most of what a licence is for, that second answer is the one that matters.

What certification opens, and what a licence alone does not.

Hospital privileges
Medical staff bylaws frequently tie privileges to certification, or to being board eligible within a stated period. Which of the two, and how long, is set by the institution.
Payer credentialing
Payers run their own criteria, separate from the state’s and from the hospital’s. A physician can hold a licence and still be working through this one.
What an employer will offer
Certification status shapes the post, the pay band and in some systems whether an offer is made at all. It is worth knowing the route before accepting a licence that constrains it.

Certification routes for a physician who did not complete an ACGME residency differ by board and by specialty, and several turn on training that has to be assessed individually.
